SUMMARY:HubCAP- Medical and Behavioral Health
DESCRIPTION:HubCAP is partnering with N.E.W Community Clinic’s Behavioral Health team to deliver non-crisis\, initial psychiatric evaluations with a Nurse Practitioner. These services will be done in the HubCAP RV\, a comfortable\, confidential and private setting equipped with couches to help you feel comfortable during your appointment. \nN.E.W Community Clinic’s Mobile unit ( MOBY) will also be onsite to accept walk in urgent care (colds\, sore throats\, minor injuries\, medicine management). N.E.W Community Clinic accepts Medicaid/Badger Care and Medicare and offers a Sliding Fee discount program for the uninsured. If you would like to save time and set up an appointment\, please call N.E.W Community Clinic’s Case Manager at 920-664-3892.\nThe HubCAP Team will be on site to provide additional resource information and connection.
