SUMMARY:HubCAP-Medical and Behavioral Health
DESCRIPTION:HubCAP is partnering with N.E.W Community Clinic’s Behavioral Health team to deliver non-crisis\, initial psychiatric evaluations with a Nurse Practitioner. These services will be done in the HubCAP RV\, a comfortable\, confidential and private setting equipped with couches to help you feel comfortable during your appointment. \nN.E.W Community Clinic’s Mobile unit ( MOBY) will also be onsite to accept walk in urgent care (colds\, sore throats\, minor injuries\, medicine management). N.E.W Community Clinic accepts Medicaid/Badger Care and Medicare and offers a Sliding Fee discount program for the uninsured. If you would like to save time and set up an appointment\, please call N.E.W Community Clinic’s Case Manager at 920-664-3892.\nThe HubCAP Team will be on site to provide additional resource information and connection. SUMMARY:HubCAP-Safe Place Parking
DESCRIPTION:The HubCAP team will be onsite at Safe Place Parking\, a program of Wise Women Gathering Place which exists to fill a gap in the homeless-to-housing continuum for people sleeping in their cars. The Safe Place Parking program provides a parking lot designed for people to sleep safely and undisturbed through the night. \nForward Service Corporation will join us on the HubCAP RV to provide resource information and FSET enrollment. Laundry Love Brown County will also be onsite to provide a meal as well as resources for laundry support. The HubCAP team will be available with additional resources and connection.
