Program/Service Description:
The Memory Assessment Center provides individuals with personalized evaluation to determine if they are affected by Alzheimer's Disease and memory loss. A team approach is utilized. During the assessment process, a social worker will speak with a family member or care giver to gather information regarding social interaction. A majority of the meeting is a sit-down conversation with the physician to complete the evaluation. The assessment will last approximately two hours. A follow-up appointment will be set up with the physician to discuss addition concerns and wrap up the initial assessment. This will last about 30 minutes and it is a family conference. Medicare and Medicaid are accepted forms of payment a well a private insurance. Dr. Tracy Hoffman is the team leader.
