Program/Service Description:
Lamers medical transport provides transportation for wheelchair or ambulatory clients for medical or social appointments. Normal transportation hours are 5:00 am to 10:00pm, seven days a week, including all holidays. Cost is $16.50 for the first five miles and $1.50 for each additional mile thereafter. For trips beyond 15 miles, please call for pricing. Medicaid recipients need to pay a co-pay of $1 for each trip except those living in nursing homes. A copy of the Medicaid card and an annual doctor certificate stating why medical transportation is needed must be on file with Lamers before transporting. Will transport on the weekends and holidays but it is encouraged to call well in advance to make arrangements. Will transport out of Brown County. Need a special order form for Medicaid recipients if the trip is over 40 miles. Generally cannot arrange for same day transportation. For private paid trips, must register with Lamers.
