Program/Service Description:
Provides geriatric consulting and care management by a licensed therapist who is a Certified Aging in Place Specialist as well as a Certified Dementia Care Specialist. Services include: comprehensive functional assessments to provide objective recommendations for the most appropriate/safest living environments and services needed to support that level of independence. Consultative services are provided for home safety and accessibility for homeowners, potential home owners, realtors, architects, contractors and developers. Surrogate "family"services are available when family cannot be present to monitor home and residential environments and situations to ensure proper health and care. Guardian Angels can also provide follow up services to ensure that living arrangements and other services are appropriate and effective for the individual. Services are provided at the location where they are needed. Private Occupational Therapy services to help with problems resulting from stroke, joint replacement, arthritis and many other health problems are available. Guardian Angels also provides educational programs for both professional and nonprofessional caregivers which are offered statewide. The consultant comes to YOU! Services are provided where the client lives. This offers the best assessment of skill and best opportunity for effective service provision. clients DO NOT need to be homebound to receive these services in their home. Fees dependent upon services.
