Program/Service Description:
The Memory Center provides an individualized memory assessment and competency evaluation to determine the extent of memory loss and Alzheimer's Disease in an individual. The assessment will help determine what physical or emotional aspects may be involved in a change of mental ability, and what services would be most beneficial. Each assessment is comprised of a detailed history and physical examination, memory testing and development of a thorough plan of care. A follow-up may be needed, depending on the individual's needs. The Memory Center is staffed by Janelle L. Cooper, MD, a board certified neurologist, a social worker, Alzheimer's Association Representative, Neuropsychology/Psychometrician, Nutritionist, and Occupational Therapy. The Memory Center is affiliated with the Alzheimer's Association of Northeastern Wisconsin, Medicare, Medicaid, and most insurance groups are accepted.
