Program/Service Description:
This is an inpatient rehabilitation program offered to the community. A person can be admitted to the program from home, a hospital, or other type of facility. Every potential patient is screened by a member of the rehabilitation team within four hours of the referral. Upon admission, each patient receives a comprehensive evaluation that takes into consideration his or her medical history and lifestyle. The results of the evaluation are used to create an individualized treatment plan. The treatment plan may focus on ambulation, muscle strengthening and coordination, use of adaptive equipment, independent living skills, cognitive and communication skills as well as others. Education is also provided to the patient and family members about fall prevention, energy conservation, pain management, and other vital techniques. Follow-up phone calls after discharge are provided to ensure progress is continuing and to address any concerns.
