Program/Service Description:
Woodhaven Manor is a 20 bed community based residential facility licensed as a class C facility by the State of Wisconsin. This facility is wheelchair accessible. There are twenty private rooms with two semi private baths. Base rate is $2,000/month with additional cost depending on care needs. Accepts Community Options and private pay for payment. Offers daily fitness activities.
