Program/Service Description:
West Care Terrace is licensed as a Class C facility by the State of Wisconsin. There are eight private rooms and baths and is wheelchair accessible. Cost is $3,100/month. Offers daily activities and van service for rides to appointments. Specializes in dementia. Staff to resident ratio is 1:4. There is 24 hour awake staff, door alarms, and individualized menus. There is no smoking. Accepts Community Options Program (COP) and private funds for payment.
