Program/Service Description:
San Luis Medical and Rehab Center is a 132 bed skilled nursing facility with 18 private and 58 semi private rooms. Included is a 24 bed Reflections Unit, which is secured and designed to meet the needs of individuals with Alzheimer's Disease and other dementia related diseases. San Luis offers daily activities, physical, occupational and speech therapies, wound care, IV therapy, respite and hospice care, religious services and a beauty/barber shop. Contracts with Unity Hospice, Southern Care, and Heartland Home Health to provide hospice care. The facility is Medicare certified and accepts Medicaid, managed care, and private insurance.
