Harbor House is a 19 bed community based residential facility licensed as a Class C facility by the state of Wisconsin. It is wheelchair accessible. Cost is $3,800/month. Accepts Community Options Program (COP), private insurance, and private funds for payment. All rooms are private. The facility focuses on clients with Alzheimer's Disease and offers support groups, alarmed doors, activities, 24 hour awake staff, Safe Harbor Program for dementia specific individuals, outings, beautician, communion service, home cooked meals, rides to appointments, laundry, walking paths, and respite care. Will accept individuals with a brain injury and challenging behaviors. Hospice available.
