Program/Service Description:
Dental Associates is a multi-specialty group practice providing total family dental care. Pediatric dentistry, oral surgery, periodontics, orthodontics, and prosthodontic specialists are on staff. DAL provides dental services to patients with private insurance or those who do not have insurance. Will take Medicaid only as a secondary insurance. (ie any patients with medical assistance must also have private insurance). Dental insurance plans are offered by CARE-PLUS DENTAL PLANS and administered by DAL. There is a Gold Plan offered for those 55 years and older who do not have insurance for $30/$50 a year, and pays 20 to 30% of the cost of the dental procedure. CP VIP is for individuals that are younger than 55 who do not have insurance. The plan costs $30/$60/$90 per year and pays 20 to 30% of costs on dental procedures. The CP Supplemental Policy is for those who have dental insurance and wish to supplement their policy. The cost is $40/$70 a year and pays co-payments and deductibles.
