Program/Service Description:
A home health care agency providing medical and non medical services. Provides such services as RN, LPN, physical therapy, occupational therapy, speech therapy, home health aides, personal care worker, companion, respite care, housekeeping, bathing, chore services, homemaker assistance, and live-in care. Cost is as follows: RN-$120/hour; home health aide-$73/visit; personal care worker-$24/hour; homemaker-$17.56/hour. There is a one hour minimum. Accepts Medicare, Medicaid, insurance, private pay, Community Options and Community Integration Program for payment. They also offer a Hmong program for older Hmongs where the home care worker is Hmong.
