Program/Service Description:
A non-medical & home health aide level home health care agency. Provides services such as personal care services, home health aide services, companionship, homemaker assistance, light housekeeping, chore service, medication reminders, meal preparation, grocery & other shopping, transportation, errands, bathing service, and respite care. Accepts private pay for payment; also able to bill Medical Assistance and private long term care insurance. Contracted provider for Outagamie & Winnebago Counties. Costs are as follows: homemaker-$16.03/hour; personal care worker-$19.07/hour; respite care/personal care level-$13.59/hour; respite care/companion care level-$11.90/hour; overnight care-$11.25/hour. There is a one hour minimum.
