Program/Service Description:
Care for All Ages is a 30 bed community based residential facility that is licensed as a Class C unrestricted facility by the State of Wisconsin. This facility is wheelchair accessible. All rooms are private. Offers kitchenettes, communal dining, daily activities, outings, children's programs and weekly communion. Cost is $2,000 to $2,300/month. Additional care cost is $275/month and is dependent on the resident's care needs. Wander guard is $50 extra. Respite care is $100/day. Accepts Community Options Program (COP), veterans, long term care insurance, or private funds for payment. Will accept individuals with a physical disability.
