Program/Service Description:
The program provides case coordination and supportive intervention to adults with severe and persistent mental illness who need assistance living in the community and people who cannot obtain services elsewhere due to lack of financial resources. The goal of case management is to decrease the need for inpatient hospitalizations and promote individual recovery. The Community Treatment case manager develops a comprehensive treatment plan and then monitors the individual's progress. Alternative living arrangements, payee services, medication monitoring and a mental health support group are examples of services. Once in this program individuals may be referred to the Comprehensive Community Service or the Community Support Program if determined eligible through a functional screen.
