Program/Service Description:
Sure Step is a falls prevention program that provides a free comprehensive in-home assessment for a person at-risk of falling. The program targets persons age 65 and older who are unable to participate in a classroom setting, have a cognitive decline, are living with a caregiver and are at risk for falls. The person's physician receives a copy of the assessment. Follow up calls are made up to a year.
