Female; Ages 18-44; No health insurance or unable to pay deductibles or copayments or insurance does not cover these services; patient of Prevea Clinic or has no current health care provider (other healthcare systems may have similar programs); Must meet income guidelines.

Intake Process:

    

Participants are screened for eligibility by calling Kathie Tilot at(920) 498-4205. Basic information is taken. An appointment is set up to finish the paperwork and to provide proof of income and identification. Prevea Clinic then calls the participant to schedule the clinical exam at a later date.

English and Spanish during enrollment process. At time of of the clincial exam,translators can be provided free of charge but this must be arranged prior to the appointment.

Program/Service Description:
St. Mary's A Woman's Place, with the help of partner Prevea Clinic, offers free reproductive cancer screenings for eligible women. The screening includes a health assessment conducted by a nurse practitioner, pelvic exam and Pap test and palpation breast exam. The cost of all services not to exceed $500. Additional procedures recommended by the health care provider or any costs exceeding $500 in a one-year period become the responsibility of the patient. Funding for the program is provided by the Marti Spittell Foundation and the Heritage Foundation.
