Children must be 3 years old or 4 years old by Sept. 1 of that current school year and meet income guidelines; 90% of applicants must be at or below federal poverty level.

Applications available in the Head Start office and at local agencies throughout Green Bay. Birth Certificate of child, proof of income for previous 12 months (or the previous year's taxes)and child immunizaton record all required. Physician will need to complete a form regarding a recent physical examination.

Program/Service Description:
Head Start is an education and socialization program for children 3 and 4 years old. There is a half day school component 4 times a week and a home visit component to the program. Parents are encouraged to become involved in program activities with their child. Both receive services in the areas of education, health, nutrition, special needs, family services and parent involvement. Head Start is an inclusion program - a percentage of the students involved have a disability.
