Program/Service Description:
Diabetes education is provided to persons with diabetes as well as their families or significant others. The purpose is to help the person earn about the disease and how to best to manage it while promoting independence in self-care. The focus is outpatient treatment where indirect and/or group education is provided in the following areas: basic knowledge, home monitoring, hypoglycemia, insulin management, nutrition, exercise, hygiene, psychological adjustment, and other information. There are 12 clinics available. Call to find the nearest location. A referral is required from your physician if you are a new patient. The clinic, once it receives the referral, will contact you to schedule an appointment.
