Program/Service Description:
The Foundation helps to bridge the gap by providing financial assistance to eligible patients to cover certain out-of-pocket costs including: prescription drug co insurance, co payments, and deductibles, health insurance premiums, other selected out-of-pocket health care costs. Focuses on patients with cancer, asthma, arthritis, and other chronic or life threatening diseases. Eligibility is based on an individual's financial, medical, and insurance situation.
