Program/Service Description:
This foundation is dedicated to assisting insured patients who cannot afford the out-of-pocket costs associated with their treatment needs. It assists the underinsured by providing financial assistance. To be eligible for assistance, patients must be a US resident and meet certain financial, insurance and medical criteria established by the Foundation's board of directors. Patients and their physicians must submit a completed application and income verification is required. Contact the Patient Access Network for an application.
