Program/Service Description:
This program pays Wisconsin health care providers for the treatment of chronic renal disease for certified chronic renal disease program recipients after all other sources of payment have been used. To be eligible you must be: 1. Diagnosed as having chronic renal disease which is irreversible and requires dialysis or a kidney transplant 2. A permanent WI resident 3. Paying Medicare Part B premiums if eligible for Medicare. Applications are available at chronic renal disease approved dialysis and transplantation facilities.
