Program/Service Description:
West Care Terrace offers two locations: 1715 and 1721 Westminster Dr. Each facility is licensed as a Class C facility by the State of Wisconsin. There are eight private rooms and baths in each facility. Each facility is wheelchair accessible. Cost is $3,170/month. Offers daily activities and van service for rides to appointments. Specializes in dementia. Staff to resident ratio is 1:4. There is 24 hour awake staff, door alarms, and individualized menus. There is no smoking. Accepts Community Options Program (COP) and private funds for payment.
