Program/Service Description:
This transportation service is equipped to accommodate ambulatory and non-ambulatory riders. Drivers are trained in first aid, CPR and seizure management. Appointments are available for hospital discharges, inter-hospital transfers, residence to doctor appointments, medical services and non-medical social engagements. Parties are billed for transportation directly. Clients residing in private residences and Community Based Residential Facilities must pay a co-pay of $1 each way if they are a Medicaid (MA) recipient. Clients who are a Medicaid (MA) recipient and residing in a nursing home do not need to pay the co-pay. Cost is $17.50 for the first five miles and $1.75 for every mile over five. Hospital discharge is $36 for the first five miles and $1.75 for every mile over five. Will provide transportation on Saturday and Sunday by appointment.
