Program/Service Description:
Professional Care Management Service. Next Stage Planning conducts a comprehensive assessment process to develop a written plan of care that offers specific recommendations to trusted service providers based on the person's needs and preferences. Other services offered are information about possible funding sources, coordination of agreed upon services, assistance with finding the appropriate alternative housing arrangement and thus assisting with the move, ongoing visits to monitor for changes, act as a family liaison and advocate.
