$1.75/mile plus base rate of $20 for private pay. With Paratransit Card, $3.00 for curb to curb service and $5.00 for door to door service. Accepts Medicaid for medical appointment transportation, with $1 copay for those who live in their own home or apartment.

Program/Service Description:
Provides transportation services for individuals. Can also provide medical transportation such as to doctor and dentist appointments. Accepts the Paratransit Card from older adults, which is purchased at Green Bay Transit, 901 University Ave. Paratransit applications are available at the Aging and Disability Resource Center of Brown County, 300 S Adams. This card allows older adults to ride at a discounted rate. Basic cost of transportation with the Paratransit Card is $3.00 for curb to curb service or $5.00 for door to door service. Cost is $1.75/mile plus the base rate of $20 for privately paid transportation. Accepts Medicaid for payment which covers Medicaid covered services. There is a $1 copay for people who live in their own home or an apartment. Call one day ahead to schedule a ride. Same day transportation can be scheduled. Sunday transportation available by appointment. Out of county transportation available. Will transport to Madison, Milwaukee and Marshfield. Closed on major holidays.
