Program/Service Description:
Lutheran Social Services' Hmong Elder Program works with other community organizations to provide services to Hmong elders. The goals of the Hmong program are to educate older adults about physical and mental health issues, nutrition and community resources, and to offer social outlet opportunities through community activities. These activities include: Hmong Elder Women's Support Group, educational trips and presentations, picnics, meeting new people, holiday celebrations, family gatherings and others. Outreach services are also offered by a staff that includes Hmong individuals. These services include: home visitations for support or other assistance, rides and accompaniment to a doctor or hospital, education about Hmong Elder Program and other community services.
