Program/Service Description:
There are two homes with 15 beds each and are licensed as class C community based residential facilities by the State of Wisconsin. There are 13 private rooms, two couple rooms with half bath and one large private with half bath in each facility. Base rate for building A is $2,425-$4,050/month. Base rate for building B is $2,625-$4,250. Cost is dependent upon the size of the room. Both buildings accept Community Options Program (COP), long tem care insurance, Veterans benefits and private funds for payment. Building B is Alzheimer specific. There is an alarm system on all doors, 24 hour awake staff, activities and outings.
