Program/Service Description:
This facility is licensed as a Class C community based residential facility by the State of Wisconsin. It has 19 private rooms with a half bath in each room. Base rate is $2,540/month which includes cable television and telephone. There are daily activities, outings, rides to appointments, house cleaning and laundry services, and an emergency call system. It is wheelchair accessible. Accepts private funds for payment and Community Options Program (COP).
