Cedar Creek Adult Care Home is a 12 bed community based residential facility that is licensed as a Class AS facility by the State of Wisconsin. There are 12 private rooms with 5 semi-private bathrooms. This CBRF offers crafts, daily activities, rides to appointments and religious services. Cost is $2,898/month. Accepts Community Options Program (COP) and private funds for payment. Will accept individuals with a developmental disability, mental illness, alcohol and drug abuse problems, and cognitive impairments.
