Program/Service Description:
A home health care agency that provides medical and non medical services. Services include: RN, LPN, Personal Care Workers, Home Health Aides, homemakers, respite care, housekeeping, assistance with bathing, chore services, and homemaker assistance. Available 24 hours a day, 7 days a week. Accepts Medicare, Medicaid, Community Options Program (COP), Community Integration Program (CIP), Workers Compensation, Insurance, and private funds for payment. Serves Brown, Door, Oconto, Kewaunee, and Shawano counties. Costs are: RN--$40/hour or $110/visit; LPN--$30/hour or $110 per visit; home health aide--$22/hour or $90/visit; personal care worker--$17.75/hour; home care attendant--$14.50/hour. There is a 1 1/4 hour minimum; 1/4 hour travel time is charged. Mileage is .40 per mile.
