Program/Service Description:
This is an assessment and service planning program for persons with a developmental disability/physical disability and their families. Services are directed at community integration as an alternative to institutional care. Supportive home care, daily living skills training, vocational services, respite, adult family homes and other services are available within the limits of available funds. Waiting lists may apply.
