Program/Service Description:
Birch Creek by Hillcrest is licensed by the State of Wisconsin as an 84 bed Class C Community Based Residential Facility (CBRF). There are 68 private suites and 16 semi-private suites. It accepts residents who are ambulatory, semi-ambulatory and non-ambulatory. Meals, laundry and room cleaning services are provided. There is 24 hour awake nursing staff and an emergency response system in the rooms. Assistance is provided with showers/baths, dressing, and general activities of daily living. Activities available in-house and outside of facility. Cost ranges from $3,276 to $4,575 depending on the size of the apartment and $5,241 for couples. Accepts Community Options for payment and Medicaid waiver program.
