Program/Service Description:
A home health care agency that provides medical and non-medical services which are available 24 hours a day, 7 days a week. Accepts Medicare, Medicaid, private insurance, Community Options Program (COP), worker's compensation, Katie Beckett, Community Integration Program (CIP), and private funds for payment. Offers foot care in the community as well. Costs are as follows: skilled nurse--$142/visit; private duty nurse--$54; home health aide--$29/hr; personal care worker--$22/hr. There is a one-hour minimum. Also offers the HomMed Home Telemonitoring System. This system will collect data such as heart rate, blood pressure, blood glucose levels, lung capacity, and weight and transmit them to a physician's office for more accurate treatment. The home monitor is small and compact. People who are cognitively impaired, have a behavioral problem, or are unsafe at home are not recommended for the program. Medicare eligible patients who are referred to Bellin Home Health and meet the selection criteria will be monitored at no cost to the patient. Also available is the Community Cares Services program. A registered nurse will make home visits to draw blood (protime,CBC, lab work, etc.), do footcare, review medications, perform assessments of needs, and provide information about community resources. The cost is $50/hour with a one hour minimum. There is no billing of Medicare or private insurance with this program.
