Program/Service Description:
American Health Assistance Foundation funds the Alzheimer's Family Relief Program which helps ease the financial burden faced by families dealing with Alzheimer's Disease. Through this program, eligible applicants can receive grants to cover such costs as short-term nursing care, respite and adult day care, medications and medical supplies, living expenses, personal hygiene supplies and other costs directly related to a patient's care. The grant application must include statements from the physician, health care professional or social worker in order to be considered by the review committee. The statement must be on the professional's letterhead confirming that the patient has been diagnosed with Alzheimer's Disease, accompanied with a completed application form. Applications are funded based on established need and on a first-come basis. Applicants cannot have cash assets exceeding a certain amount. You can apply more than once and become eligible for additional assistance ninety days after the preceding grant has been awarded.
